Roles & Responsibilities

• Provide compassionate direct patient care including bathing, dressing, and assistance with activities of daily living
• Take and record vital signs accurately and report changes to the nursing team
• Help with safe transfers and mobility to promote patient comfort and safety
• Assist with feeding and nutrition needs while observing dietary instructions
• Document care and observations in the patient record clearly and promptly
• Communicate effectively with patients, families, and interdisciplinary team members to support care plans
• Maintain a clean, safe environment and follow infection control practices

Qualifications

• Current CNA certification (required) and active registration where applicable
• Basic Life Support (BLS/CPR) certification required or willing to obtain
• High school diploma or GED preferred
• Strong communication skills and the ability to work well in a team
• Ability to pass facility background checks and required health screenings
• Prior experience in long-term care or similar settings is a plus

Education

• High school diploma or GED
• State CNA certification (required)

Knowledge & Skills

• Comfortable assisting with ADLs and basic patient hygiene
• Proficient at taking and recording vital signs and observing patient condition
• Basic knowledge of infection control and safe handling techniques
• Good documentation skills and attention to detail
• Empathy, patience, and the ability to connect with patients and families
• Time management and the ability to prioritize patient needs
