Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans that improve mobility and independence
- Deliver evidence-based interventions to strengthen, stabilize, and restore function
- Complete evaluations, progress notes, and discharge documentation in line with facility and regulatory standards
- Collaborate with interdisciplinary team members to coordinate patient-centered care
- Educate patients and families on therapy goals, safety precautions, and home exercise programs to promote lasting recovery
- Maintain compliance with state, federal, and facility policies while advocating for high-quality rehab services
Qualifications
- Active California Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- Skilled Nursing Facility or rehabilitation experience preferred
- Strong communication, clinical judgment, and documentation skills
- Ability to work independently and as part of a multidisciplinary team
Education
- Graduate of an accredited Physical Therapy program
Knowledge & Skills
- Skilled in physical therapy evaluation and treatment planning
- Knowledge of rehabilitative interventions to improve mobility, strength, balance, and function
- Proficient in clinical documentation and regulatory compliance
- Excellent interdisciplinary collaboration and patient/family education skills
- Strong clinical reasoning and time management in a skilled nursing setting
