Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (day) with some weekends and on-call required

Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Assess patients and caregivers using the nursing process to deliver compassionate, evidence-based hospice care
- Develop and communicate realistic, individualized plans of care that include patients and families
- Serve as Case Manager — actively manage caseloads and visits to meet agency productivity and quality standards
- Complete timely, accurate OASIS and agency documentation in accordance with state and federal regulations
- Implement medical plans of care and nursing interventions to ensure patient safety and symptom control
- Perform and maintain high-tech skills such as IV therapy, wound VAC management, and complex wound care
- Coordinate safe discharges, transfers, and interdisciplinary care with physicians, therapists, social work, and other team members
- Provide clear patient and caregiver education on medications, treatment options, discharge instructions, and reportable conditions
- Collaborate with LPNs and other staff—delegate appropriately and review documentation to ensure care continuity
- Participate in peer review audits, case conferences, and IDT meetings to continuously improve care quality
Qualifications
- Active Registered Nurse (RN) license in Vermont (Vermont State Board of Nursing) — required
- Experience or comfort with hospice/home health nursing and case management
- Proficient with OASIS documentation and agency-required documentation timeliness
- Skilled in IV therapy, wound VAC care, and complex wound management
- Familiarity with infection control standards and ability to apply them in home settings
- Experience coordinating care with interdisciplinary teams and physicians
- Comfort with occasional weekend work and on-call responsibilities
Knowledge & Skills
- Patient assessment and individualized care plan development
- Symptom and pain management tailored to hospice needs
- Timely, accurate documentation (OASIS and agency systems like McKesson)
- Case management and visit time management
- Infection control best practices in home care environments
- Effective patient/caregiver education and communication
- Coordination of discharge planning and community resources
- Comfort with electronic charting workflows and follow-up processes
