Roles & Responsibilities
- Perform thorough home health OT assessments, complete OASIS documentation, and develop individualized plans of care
- Provide skilled interventions such as gait training, mobility/transfers, assistive device training, and home exercise programs
- Deliver meaningful patient and family education to support safety, independence, and recovery at home
- Document visits and care plans accurately and efficiently using Epic and Rover, meeting agency standards
- Manage a caseload of up to 6 visits per day while maintaining expected productivity (around 30 points)
- Coordinate care with nurses, PTs, SLPs, physicians, and community resources to optimize patient outcomes
- Safely transport yourself between visits, prioritize time management, and minimize drive time through smart routing
- Identify discharge needs, complete reassessments, and ensure timely communication for continuity of care
Qualifications
- Current Occupational Therapist license in Oregon or eligible — pending licenses are accepted
- Minimum 1 year of skilled Home Health OT experience required
- BCLS (Basic Cardiac Life Support) certification required
- OASIS experience required — please highlight if you do not have OASIS experience
- Experience with Epic and Rover charting preferred
- Reliable transportation and valid driver's license required (occasional up to ~60 mile days)
- Comfort managing a productive caseload and working independently in patient homes
- Strong communication skills and ability to collaborate with interdisciplinary teams
Education
- Master's in Occupational Therapy (MSOT) or OTD preferred; OTR credential required or in progress
Knowledge & Skills
- Skilled in assessment, documentation, discharge planning, and Plan of Care development
- Proficient with OASIS and Epic charting workflows
- Experienced in gait training, mobility/transfers, Hoyer lift use, and adaptive equipment
- Skilled in patient and family education and creating effective home exercise programs
- Familiar with orthopedics, geriatrics, wound care, neuro rehab techniques (including NDT), and common modalities like e-stim, TENS, ultrasound, and moist heat
- Ability to triage clinical priorities, manage time, and meet productivity expectations
- Comfort with Medicare/Medicaid/Commercial billing contexts and documentation requirements
