Roles & Responsibilities

• Assess assigned patients thoroughly and document clinical findings
• Develop individualized care plans that reflect patient needs and goals
• Implement nursing interventions, including safe chemotherapy/biotherapy administration when indicated
• Evaluate patient responses and update plans of care to ensure positive outcomes
• Provide leadership and supervision to CNAs/SNAs and other care team members
• Coordinate care with interdisciplinary teams and community resources
• Educate patients and families about treatment, home care, and safety
• Maintain accurate clinical documentation and comply with agency and regulatory standards
• Adhere to infection control practices and agency COVID policies

Qualifications

• Active Illinois nursing license required (must include copy with submission)
• Minimum 1+ years oncology nursing experience required
• ONS or ONCC certification preferred/required per profile
• Active Chemotherapy/Immunotherapy and Chemotherapy/Biotherapy Infusion Certifications required
• BLS (AHA) certification required
• Valid driver’s license and active car insurance required
• Certifications and license must be active at time of submission
• Submission packet: resume, skills checklist (within 1 year), 2 supervisory references, COVID card/status

• Patient assessment and clinical judgment in oncology home health
• Care planning, implementation and outcome evaluation
• Home health nursing practices and documentation
• Leadership and supervision of direct care staff
• Safe chemotherapy/biotherapy infusion practices
• Patient and family education, including discharge and home instructions
• Infection control and adherence to agency protocols
