Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents across daily routines and acuity levels
- Administer medications and treatments accurately and safely according to facility protocols
- Monitor and document vital signs, resident status, and response to interventions in the medical record
- Perform wound care, basic clinical procedures, and timely reporting of changes in condition
- Collaborate closely with RNs, CNAs, therapists, and families to deliver person-centered care
- Educate residents and families about care plans and daily self-care needs in an approachable way
- Maintain a safe, clean environment and follow infection control and safety standards
Qualifications
- Valid LPN license (Licensed Practical Nurse) — active and in good standing
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Prior LPN experience in long-term care, skilled nursing, or similar settings is preferred
- Ability to work evenings and weekends as required by the assignment
- Up-to-date immunizations and ability to pass background and health screenings
- Strong interpersonal skills and a team-minded attitude
Education
- LPN diploma, vocational nursing certificate, or equivalent practical nursing education
- High school diploma or GED
Knowledge & Skills
- Sound knowledge of medication administration and documentation best practices
- Skilled in resident assessment, vital signs, and recognizing changes in condition
- Familiarity with long-term care routines, wound care, and mobility assistance
- Comfortable using electronic medical records and charting accurately
- Excellent communication, time management, and teamwork skills
- Ability to prioritize workload in a fast-paced care setting
