Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents across daily routines and acuity levels
• Administer medications and treatments accurately and safely according to facility protocols
• Monitor and document vital signs, resident status, and response to interventions in the medical record
• Perform wound care, basic clinical procedures, and timely reporting of changes in condition
• Collaborate closely with RNs, CNAs, therapists, and families to deliver person-centered care
• Educate residents and families about care plans and daily self-care needs in an approachable way
• Maintain a safe, clean environment and follow infection control and safety standards

Qualifications

• Valid LPN license (Licensed Practical Nurse) — active and in good standing
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Prior LPN experience in long-term care, skilled nursing, or similar settings is preferred
• Ability to work evenings and weekends as required by the assignment
• Up-to-date immunizations and ability to pass background and health screenings
• Strong interpersonal skills and a team-minded attitude

Education

• LPN diploma, vocational nursing certificate, or equivalent practical nursing education
• High school diploma or GED

Knowledge & Skills

• Sound knowledge of medication administration and documentation best practices
• Skilled in resident assessment, vital signs, and recognizing changes in condition
• Familiarity with long-term care routines, wound care, and mobility assistance
• Comfortable using electronic medical records and charting accurately
• Excellent communication, time management, and teamwork skills
• Ability to prioritize workload in a fast-paced care setting
