Roles & Responsibilities

• Provide high-quality, compassionate nursing care to long-term care residents during night shift
• Administer medications and treatments accurately and document timely
• Perform resident assessments and communicate changes in condition to the interdisciplinary team
• Support safe transitions of care and assist with admissions, discharges, and transfers as needed
• Participate in care planning and assist with MDS-related tasks when applicable
• Follow facility policies and regulatory requirements to ensure resident safety and compliance

Qualifications

• Current, unrestricted Colorado LPN license (required)
• Long Term Care (LTC) experience (required)
• Night shift availability (required)
• MDS experience (preferred)
• Ability to pass facility onboarding and any required background checks

Education

• Graduation from an accredited LPN / Practical Nursing program and active LPN licensure in Colorado

Knowledge & Skills

• Strong medication administration and documentation skills
• Knowledge of long-term care resident needs and best practices
• Familiarity with MDS processes and LTC regulatory requirements (preferred)
• Good clinical assessment and prioritization skills during overnight hours
• Effective communication and teamwork skills with clinical and support staff
