Roles & Responsibilities

• Develop and implement individualized case management plans in collaboration with patients, families, physicians and the care team
• Complete comprehensive assessments and determine eligibility for case management using clinical judgment
• Monitor patient progress toward goals, document interventions, and revise plans as needed
• Coordinate discharge planning to ensure safe transitions and continuity of care
• Facilitate integration of behavioral health and long-term services and supports for whole-person care
• Manage an ongoing caseload with regular outreach and timely follow-up
• Communicate clearly with multidisciplinary teams, patients and caregivers to remove barriers to care
• Maintain accurate EMR documentation and meet regulatory and organizational education requirements

Qualifications

• Current Montana Registered Nurse (RN) license required
• American Heart Association BLS certification required
• Minimum of 2 years of nursing experience required
• Utilization review experience is required
• Bachelor's Degree in Nursing (BSN) preferred
• 2 years of acute care hospital experience as a case manager preferred
• Comfort with on-call rotations and after-hours responsiveness
• Strong interpersonal, verbal and written communication skills

Education

• Registered Nurse (RN) licensure required
• Bachelor's Degree in Nursing (BSN) preferred

Knowledge & Skills

• Case management and care coordination best practices
• Utilization review processes and criteria
• Patient assessment and discharge planning
• Electronic Medical Records (EMR) and MS Office proficiency
• Excellent documentation and clinical decision-making skills
• Ability to manage multiple priorities and deadlines
• Emotional intelligence, tact, discretion and confidentiality
• Familiarity with behavioral health integration and long-term services and supports
