Roles & Responsibilities

• Evaluate patients comprehensively and develop individualized treatment plans to restore function and independence
• Deliver evidence-based therapeutic interventions, including mobility, balance, strength, and gait training
• Collaborate closely with nurses, occupational therapists, speech therapists, and physicians to coordinate care
• Document patient progress, goals, and discharge plans clearly and timely in the medical record
• Educate patients and families on home exercise programs, safety, and strategies to maintain progress
• Contribute to interdisciplinary discharge planning to ensure safe transitions and optimal outcomes

Qualifications

• Current Physical Therapist license (eligible to practice in Massachusetts preferred)
• BLS/CPR certification (or willingness to obtain upon hire)
• Experience in skilled nursing, inpatient rehab, or post-acute care settings preferred
• Strong clinical reasoning, time management, and patient-family communication skills
• Ability to work collaboratively in an interdisciplinary team environment

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's in a related field with appropriate licensure considered

Knowledge & Skills

• Proven skills in assessment, therapeutic exercise, manual techniques, and mobility training
• Comfortable with documentation and care planning in an electronic medical record
• Knowledge of post-acute care workflows and regulatory expectations
• Excellent communication, patient education, and teamwork abilities
