Roles & Responsibilities
- Perform comprehensive patient evaluations to assess functional performance, self-care skills, and rehabilitation needs
- Develop and implement personalized treatment plans tailored to each patient’s goals and clinical needs
- Provide therapeutic activities, exercises, and adaptive strategies to maximize independence
- Monitor and document patient progress while ensuring compliance with state and federal guidelines
- Educate patients and families on therapy goals, adaptive equipment, and home exercise programs
- Collaborate with the interdisciplinary healthcare team to coordinate high-quality patient care
Qualifications
- Active and current Occupational Therapist (OT) license in the state of California (required)
- Graduate of an accredited Occupational Therapy program (OTD or equivalent)
- Previous experience in a Skilled Nursing Facility (SNF) required
- Strong clinical knowledge with a patient-centered approach
- Excellent communication, organization, and teamwork skills
Education
- OTD or equivalent from an accredited Occupational Therapy program

- Patient and family education
- Interdisciplinary collaboration with PTs, nurses, physicians, and SLPs
