Roles & Responsibilities
- Assess patients' functional performance and rehabilitation needs with thoughtful evaluations
- Develop personalized treatment plans that prioritize patient goals and independence
- Implement therapeutic activities and adaptive strategies to maximize daily living skills
- Monitor and document patient progress following state and federal guidelines
- Educate patients and families on therapy goals, home programs, and adaptive equipment
- Collaborate closely with nurses, PTs, physicians, speech therapists, and the interdisciplinary team to coordinate care
Qualifications
- Active and current Occupational Therapist (OT) license in the state of practice (required)
- Graduate of an accredited Occupational Therapy program (OTD or equivalent)
- Previous experience in a Skilled Nursing Facility (SNF) is required
- Strong clinical knowledge with a patient-centered approach
- Excellent communication, organization, and teamwork skills
