Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing facility
• Administer medications and treatments accurately and document using facility systems
• Perform routine patient assessments and communicate changes promptly to the care team
• Assist with activities of daily living while promoting resident dignity and independence
• Collaborate with RNs, CNAs, therapists and family members to create effective care plans
• Support safe admission, transfer and discharge processes with clear handoffs
• Participate in quality improvement and infection control practices to keep residents safe

Qualifications

• Valid New York LPN license (required)
• Current BLS/CPR certification (required)
• Active immunizations and TB screening per facility policy (required)
• Experience in long term care or skilled nursing preferred
• Strong clinical assessment and medication administration skills
• Ability to pass background check and meet facility credentialing requirements
• Excellent communication skills and a collaborative nursing approach

Education

• Completion of an accredited Practical Nursing program (LPN diploma or equivalent)

• Medications and dosage calculations
• Wound care and basic dressing management
• Vital sign monitoring and resident assessment
• Electronic health record charting and documentation
• Care planning and interdisciplinary teamwork
• Infection prevention and safe handling techniques
• Time management and prioritization in a busy SNF setting
