Roles & Responsibilities

• Deliver compassionate, patient-centered nursing care to elderly residents in a skilled nursing setting
• Perform timely assessments and create clear, actionable care plans
• Administer medications and treatments accurately while monitoring resident response
• Collaborate with physicians, therapists, and care staff to coordinate safe, effective care
• Document care and resident progress in the electronic medical record with attention to detail
• Advocate for resident safety and dignity, promoting quality of life and independence
• Educate residents and families about care plans, medications, and discharge or transition needs
• Participate in quality improvement initiatives and help maintain a positive, team-focused environment

Qualifications

• Active Registered Nurse (RN) license for New York (required)
• Basic Life Support (BLS) certification required; ACLS or other advanced certifications are a plus
• Experience in skilled nursing facility (SNF) or geriatric care preferred
• Comfortable with medication administration, wound care basics, and chronic condition management
• Strong communication skills and ability to work collaboratively with interdisciplinary teams
• Proof of immunizations and compliance with facility onboarding requirements

Education

• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Geriatric care principles and person-centered approaches
• Skilled nursing facility workflows and regulatory awareness
• Patient assessment and care planning for older adults
• Medication administration and documentation best practices
• Familiarity with electronic medical records and clinical documentation
• Strong clinical judgment, prioritization, and time management
