Shift Type: Per diem / Open schedule, hours vary by assignment; may include day, evening, and night shifts and weekends (shifts assigned as needed)

Roles & Responsibilities
- Deliver compassionate, patient-centered nursing care to elderly residents in a skilled nursing setting
- Perform timely assessments and create clear, actionable care plans
- Administer medications and treatments accurately while monitoring resident response
- Collaborate with physicians, therapists, and care staff to coordinate safe, effective care
- Document care and resident progress in the electronic medical record with attention to detail
- Advocate for resident safety and dignity, promoting quality of life and independence
- Educate residents and families about care plans, medications, and discharge or transition needs
- Participate in quality improvement initiatives and help maintain a positive, team-focused environment
Qualifications
- Active Registered Nurse (RN) license for New York (required)
- Basic Life Support (BLS) certification required; ACLS or other advanced certifications are a plus
- Experience in skilled nursing facility (SNF) or geriatric care preferred
- Comfortable with medication administration, wound care basics, and chronic condition management
- Strong communication skills and ability to work collaboratively with interdisciplinary teams
- Proof of immunizations and compliance with facility onboarding requirements
Education
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Geriatric care principles and person-centered approaches
- Skilled nursing facility workflows and regulatory awareness
- Patient assessment and care planning for older adults
- Medication administration and documentation best practices
- Familiarity with electronic medical records and clinical documentation
- Strong clinical judgment, prioritization, and time management
