Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents
• Administer medications and treatments accurately per provider orders
• Assess and monitor vital signs and report clinical changes promptly
• Document care and observations thoroughly in the electronic medical record
• Collaborate with the interdisciplinary team to support individualized care plans
• Educate residents and family members about care, medications, and safety
• Maintain infection control, safety standards, and a clean care environment

Qualifications

• Current, unrestricted New York State LPN license (required)
• BLS/CPR certification (required) and willingness to obtain facility-required certifications
• Experience in long-term care or similar settings is a plus
• Strong communication and teamwork skills with a patient-first mindset
• Ability to pass background check and drug screen as required
• Reliable transportation or willingness to travel locally for shifts

Education

• Graduate of an accredited LPN/Vocational Nursing program; high school diploma or GED required

Knowledge & Skills

• Medication administration and documentation
• Basic patient assessment and wound care knowledge
• Familiarity with electronic medical records (EMR) and charting
• Time management and prioritization in a fast-paced environment
• Ability to follow care plans and safety/infection control protocols
• Collaborative approach to working with nursing and support staff
