Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Administer medications and treatments safely and accurately
• Monitor and document vital signs, changes in condition, and interventions
• Assist residents with activities of daily living while promoting independence
• Communicate effectively with residents, families, and the interdisciplinary care team
• Maintain accurate and timely clinical documentation in the resident chart
• Adhere to infection control standards and facility policies to keep everyone safe
• Respond to resident needs and clinical emergencies with professionalism and calm

Qualifications

• Active Licensed Practical Nurse (LPN) license in New York required
• Current CPR/BLS certification required
• Experience in long-term care or skilled nursing preferred
• Up-to-date immunizations and ability to pass background check and drug screen
• Comfortable working independently on per-diem assignments and collaborating with facility staff
• Professional, compassionate bedside manner and strong communication skills

Education

• Vocational nursing diploma or equivalent and completion of an accredited LPN program
• Current LPN license in New York

Knowledge & Skills

• Medication administration and documentation best practices
• Basic assessment skills and ability to recognize changes in condition
• Familiarity with long-term care workflows and resident-centered care
• Good charting skills; ability to document clearly and accurately
• Teamwork, time management, and adaptability to variable shift needs
• Infection prevention and safety practices
