Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the Clifton Hills unit
• Administer medications and treatments accurately and safely
• Monitor and record vital signs, report changes in condition to the care team
• Assist residents with activities of daily living and promote independence
• Collaborate with RNs and interdisciplinary team to implement individualized care plans
• Document nursing assessments and care in the facility EMR in a timely manner
• Support a positive, respectful environment that values resident dignity and quality of life

Qualifications

• Valid LPN license for New York State required
• Graduate of an accredited practical nursing program
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Minimum experience in long-term care or skilled nursing preferred
• Ability to pass background checks and meet facility immunization requirements
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited practical nursing program; LPN diploma or equivalent

Knowledge & Skills

• Knowledge of long-term care practices and regulations
• Skill in medication administration and wound care basics
• Comfortable using electronic medical records for documentation
• Excellent observational skills for changes in resident condition
• Good interpersonal skills to interact with residents, families, and staff
