Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Administer medications and treatments accurately and document in the resident record
• Monitor residents for changes in condition and promptly report concerns to the care team
• Collaborate with RNs, CNAs and interdisciplinary staff to create and follow individualized care plans
• Assist with admissions, transfers and discharges to ensure continuity of care
• Maintain a safe, clean environment and follow infection control and safety protocols
• Communicate clearly and kindly with residents and families, providing education and emotional support

Qualifications

• Current LPN license in New York state or multistate license where applicable
• Completion of an accredited practical nursing program
• Current BLS/CPR certification at time of hire
• Minimum practical nursing experience in long-term care or similar setting preferred
• Up-to-date immunizations and ability to pass background and health screenings
• Strong interpersonal skills and a resident-first mindset

Education

• Graduation from an accredited practical nursing program; LPN/LVN certificate required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic clinical assessment and vital signs monitoring
• Familiarity with long-term care workflows and care plan implementation
• Electronic health record (EHR) documentation skills
• Infection control, wound care basics and safe patient handling
• Clear verbal and written communication with residents and clinical teams
