Roles & Responsibilities

• Provide compassionate, resident-centered care and assist with activities of daily living
• Assist with bathing, dressing, grooming and toileting while preserving resident dignity
• Take and record vital signs and report any changes in condition to nursing staff
• Support residents with mobility and transfers using safe body mechanics
• Follow infection control protocols and maintain a clean, safe environment
• Complete and maintain accurate documentation of care and observations
• Participate in competency testing and skill validations as required by the facility
• Communicate effectively with residents, families and the interdisciplinary team to support care goals

Qualifications

• Current Certified Nursing Assistant (CNA) certification (New York State) required
• BLS (Basic Life Support) certification required
• License/Certificate verification and ability to pass Trinity Specialty Competency Testing
• Ability to pass required health screenings: Trinity 10-panel drug screen, fit test, 2-step PPD or IGRA
• Up-to-date immunizations per facility policy (COVID-19, MMR, Hep B, Influenza, Varicella, Tdap)
• Physical/Health Statement confirming ability to perform required duties
• Strong communication skills, reliability, and a compassionate patient-first attitude

Education

• Completion of an approved CNA training program
• High school diploma or GED preferred

Knowledge & Skills

• Hands-on experience with ADLs and resident hygiene
• Ability to take and document vitals and observe/report clinical changes
• Infection control practices and PPE usage (including fit-tested respirators)
• Safe transfer and mobility assistance techniques
• Basic charting and documentation skills
• Teamwork, time management, and respectful resident interaction
• Ability to follow care plans and facility protocols
