Roles & Responsibilities
- Provide compassionate direct nursing care to geriatric residents, focusing on dignity and comfort
- Administer medications and treatments accurately and document care clearly
- Perform thorough patient assessments and communicate changes to the care team
- Support residents with activities of daily living while promoting independence
- Follow infection control practices and maintain a safe care environment
- Participate in Trinity specialty competency testing and facility onboarding requirements
- Collaborate with nurses, therapists, and physicians to develop and follow care plans
Qualifications
- Current LPN license in New York (license verification required)
- Basic Life Support (BLS) certification required
- Completion of Trinity specialty specific competency testing required
- Up-to-date immunizations and health screening per facility policy (COVID-19, MMR, Hep B, TB testing or IGRA, flu, varicella, Tdap, drug screen, fit test)
- Previous experience in geriatric or long-term care nursing preferred
- Strong communication skills and ability to work as part of an interdisciplinary team
Education
- Graduate of an accredited practical nursing program
- Current Licensed Practical Nurse (LPN) credential

- Medication administration and accurate charting
- Geriatric nursing principles and best practices
- Basic life support and emergency response skills
- Infection prevention and control procedures
- Patient assessment and reporting clinical changes
- Time management and prioritization in a busy care setting
