Roles & Responsibilities

• Provide compassionate direct patient care and daily living assistance to residents
• Assist with activities of daily living — bathing, dressing, feeding and mobility support
• Take and record vital signs accurately and report changes to the nursing team
• Document care thoroughly and courteously in resident records
• Support safe patient transfers and use proper body mechanics to prevent injury
• Collaborate with nurses and interdisciplinary team members to promote resident comfort and dignity

Qualifications

• Certified Nursing Assistant (CNA) certification in good standing
• Current BLS (Basic Life Support) certification
• Successful completion of Trinity Specialty Specific Competency Testing (or willingness to complete)
• License/certificate verification completed and up to date
• Ability to pass required health screenings: Fit Test, Trinity 10-panel drug screen, 2-step PPD or IGRA (Quantiferon), color vision
• Up-to-date immunizations per facility policy: COVID-19 vaccine, MMR, Hepatitis B, Influenza, Varicella, Tdap
• Physical/health statement demonstrating ability to perform essential duties

Education

• High school diploma or GED preferred
• Current CNA certification required

Knowledge & Skills

• Hands-on resident care and ADL assistance
• Infection control practices and safe PPE use
• Clear, respectful communication with residents, families, and team members
• Basic clinical observation and reporting (vital signs, behavior changes)
• Comfort with competency testing and following facility protocols
• Teamwork, reliability, and time management in a fast-paced care setting
