Roles & Responsibilities
- Deliver compassionate, hands-on nursing care to residents, prioritizing dignity and comfort
- Administer medications and treatments accurately and document in the medical record
- Support rehabilitation plans by collaborating with therapists and nursing staff
- Monitor resident condition, report changes promptly, and escalate concerns to clinical leadership
- Maintain clear, timely documentation to support quality care and regulatory compliance
- Participate in interdisciplinary team meetings to coordinate resident-centered care
- Mentor and guide support staff when assisting with daily resident needs and activities
Qualifications
- Active LPN license in New York State (required)
- CPR/BLS certification (current)
- Experience in long-term care, rehabilitation, or skilled nursing preferred
- Comfort with medication administration, resident assessments, and clinical documentation
- Strong communication skills and the ability to work collaboratively in a team
- Ability to pass facility-required health screenings and background checks
Education
- Graduate of an accredited Practical Nursing (LPN) program or equivalent diploma
Knowledge & Skills
- Foundational nursing knowledge in patient assessment, wound care, and medication safety
- Familiarity with rehabilitation practices and supporting therapy goals
- Competence with electronic medical records and timely charting
- Infection control principles and safe clinical practice
- Good clinical judgment, prioritization, and time management skills
