Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents, prioritizing dignity and comfort
• Administer medications and treatments accurately and document in the medical record
• Support rehabilitation plans by collaborating with therapists and nursing staff
• Monitor resident condition, report changes promptly, and escalate concerns to clinical leadership
• Maintain clear, timely documentation to support quality care and regulatory compliance
• Participate in interdisciplinary team meetings to coordinate resident-centered care
• Mentor and guide support staff when assisting with daily resident needs and activities

Qualifications

• Active LPN license in New York State (required)
• CPR/BLS certification (current)
• Experience in long-term care, rehabilitation, or skilled nursing preferred
• Comfort with medication administration, resident assessments, and clinical documentation
• Strong communication skills and the ability to work collaboratively in a team
• Ability to pass facility-required health screenings and background checks

Education

• Graduate of an accredited Practical Nursing (LPN) program or equivalent diploma

Knowledge & Skills

• Foundational nursing knowledge in patient assessment, wound care, and medication safety
• Familiarity with rehabilitation practices and supporting therapy goals
• Competence with electronic medical records and timely charting
• Infection control principles and safe clinical practice
• Good clinical judgment, prioritization, and time management skills
