Roles & Responsibilities

• Assess patients' functional abilities and develop individualized treatment plans focused on measurable outcomes.
• Deliver evidence-based therapeutic interventions including exercise, manual therapy and gait training.
• Document patient evaluations, progress notes and discharge summaries accurately and in a timely manner.
• Collaborate closely with nurses, physicians and rehab team members to coordinate comprehensive care.
• Educate patients and caregivers on home exercise programs, safety strategies and strategies to maximize independence.
• Monitor and adapt treatment plans based on patient response, goals and safety considerations.
• Participate in quality improvement initiatives to improve patient outcomes and clinic efficiency.

Qualifications

• Active California license as a Physical Therapist (required).
• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (preferred).
• BLS/CPR certification (current) or willingness to obtain upon hire.
• Comfort working in a rehabilitation/contract setting; previous outpatient or post-acute experience is a plus.
• Strong clinical reasoning, documentation skills and the ability to work effectively within an interdisciplinary team.

Education

• Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy

Knowledge & Skills

• Skilled in therapeutic exercise prescription, manual techniques and gait training.
• Experience with mobility assessment, balance retraining and fall prevention strategies.
• Familiarity with common rehab modalities and adaptive equipment recommendations.
• Clear communicator who can educate patients and caregivers with patience and empathy.
• Comfortable using electronic health records for timely, accurate documentation.
