Roles & Responsibilities
- Assess patients' functional abilities and develop individualized treatment plans focused on measurable outcomes.
- Deliver evidence-based therapeutic interventions including exercise, manual therapy and gait training.
- Document patient evaluations, progress notes and discharge summaries accurately and in a timely manner.
- Collaborate closely with nurses, physicians and rehab team members to coordinate comprehensive care.
- Educate patients and caregivers on home exercise programs, safety strategies and strategies to maximize independence.
- Monitor and adapt treatment plans based on patient response, goals and safety considerations.
- Participate in quality improvement initiatives to improve patient outcomes and clinic efficiency.
Qualifications
- Active California license as a Physical Therapist (required).
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (preferred).
- BLS/CPR certification (current) or willingness to obtain upon hire.
- Comfort working in a rehabilitation/contract setting; previous outpatient or post-acute experience is a plus.
- Strong clinical reasoning, documentation skills and the ability to work effectively within an interdisciplinary team.
Education
- Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy
Knowledge & Skills
- Skilled in therapeutic exercise prescription, manual techniques and gait training.
- Experience with mobility assessment, balance retraining and fall prevention strategies.
- Familiarity with common rehab modalities and adaptive equipment recommendations.
- Clear communicator who can educate patients and caregivers with patience and empathy.
- Comfortable using electronic health records for timely, accurate documentation.
