Roles & Responsibilities
- Provide skilled evaluation, individualized treatment plans, and goal-oriented therapy for patients in an outpatient/contract setting
- Deliver evidence-based interventions while documenting progress clearly and timely
- Collaborate with an interdisciplinary team to coordinate patient care and optimize outcomes
- Educate patients and families on home programs, safety, and strategies to maximize independence
- Monitor and adjust treatment plans based on patient response and functional goals
- Participate in team meetings and contribute to a positive, patient-centered culture
Qualifications
- Licensed Physical Therapist (state licensure required — facility will verify credentials)
- Experience in physical therapy practice (outpatient/rehab experience preferred)
- Basic Life Support (BLS) certification or willingness to obtain upon hire
- Strong documentation skills and experience with clinical outcome measures
- Excellent communication skills and ability to collaborate with multidisciplinary teams
Education
- Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
Knowledge & Skills
- Assessment and development of individualized PT treatment plans
- Manual therapy, therapeutic exercise, and neuromuscular re-education techniques
- Gait training, assistive device prescription, and functional mobility training
- Familiarity with common documentation systems and outcome measurement
- Patient education, safety assessment, and discharge planning
