Shift Type: Night shift, 7:00 PM - 7:30 AM (overnight). Days as scheduled / per facility need

Candidate Pay Rate: $37 per hour

Roles & Responsibilities
- Provide compassionate direct patient care and support daily living needs
- Administer medications and document treatments accurately and timely
- Monitor and record patient vital signs and report changes to the care team
- Assist with wound care, basic procedures, and restorative nursing tasks
- Maintain clear, professional charting in the resident record
- Collaborate with RNs, physicians, and interdisciplinary team members to optimize outcomes
- Support patient and family education with patience and empathy
- Contribute to a safe, clean environment by following infection control policies
Qualifications
- Licensed Practical Nurse (LPN) - active, in good standing
- Current basic life support (BLS/CPR) certification or willing to obtain
- Prior LPN experience in long-term care or skilled nursing preferred
- Strong communication skills and the ability to work well in a team
- Dependable, organized, and committed to high-quality resident care
Education
- Completion of an accredited practical nursing program and LPN licensure
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment and monitoring of vital signs
- Familiarity with long-term care workflows and resident-centered care
- Clear charting and basic clinical documentation skills
- Infection control and safe patient handling techniques
- Ability to triage and escalate clinical changes appropriately
