Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as an LPN, making their comfort and dignity a priority
• Administer medications and treatments accurately and document thoroughly
• Monitor resident conditions, report changes promptly, and collaborate with RNs and providers to adjust care plans
• Perform routine clinical tasks such as wound care, vital signs, and ADL assistance with attention to safety
• Communicate clearly and kindly with residents and families to build trust and understanding
• Participate in interdisciplinary team meetings to support coordinated, person-centered care
• Maintain accurate and timely charting in the facility’s documentation system

Qualifications

• Current Licensed Practical Nurse (LPN) credential — Pennsylvania license preferred or eligibility to obtain one
• CPR/BLS certification required (facility may accept equivalent)
• Clear background check and up-to-date immunizations as required by the facility
• Comfortable administering medications and performing typical long-term care nursing tasks
• Ability to work a 7:00 AM - 7:30 PM day shift; reliable attendance and strong teamwork skills
• Previous experience in long-term care, skilled nursing, or similar settings is a plus

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED required

Knowledge & Skills

• Solid clinical assessment skills and attention to detail
• Medication administration and documentation proficiency
• Strong communication and interpersonal skills with residents, families, and team members
• Knowledge of infection control practices and resident safety standards
• Basic electronic health record (EHR) navigation and charting
• Ability to prioritize tasks in a fast-paced environment and remain calm under pressure
