Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents as an LPN, making their comfort and dignity a priority
- Administer medications and treatments accurately and document thoroughly
- Monitor resident conditions, report changes promptly, and collaborate with RNs and providers to adjust care plans
- Perform routine clinical tasks such as wound care, vital signs, and ADL assistance with attention to safety
- Communicate clearly and kindly with residents and families to build trust and understanding
- Participate in interdisciplinary team meetings to support coordinated, person-centered care
- Maintain accurate and timely charting in the facility’s documentation system
Qualifications
- Current Licensed Practical Nurse (LPN) credential — Pennsylvania license preferred or eligibility to obtain one
- CPR/BLS certification required (facility may accept equivalent)
- Clear background check and up-to-date immunizations as required by the facility
- Comfortable administering medications and performing typical long-term care nursing tasks
- Ability to work a 7:00 AM - 7:30 PM day shift; reliable attendance and strong teamwork skills
- Previous experience in long-term care, skilled nursing, or similar settings is a plus
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED required
Knowledge & Skills
- Solid clinical assessment skills and attention to detail
- Medication administration and documentation proficiency
- Strong communication and interpersonal skills with residents, families, and team members
- Knowledge of infection control practices and resident safety standards
- Basic electronic health record (EHR) navigation and charting
- Ability to prioritize tasks in a fast-paced environment and remain calm under pressure
