Roles & Responsibilities

• Provide compassionate bedside care and daily support to residents
• Administer medications and treatments accurately and on time
• Monitor, document, and report changes in patient condition clearly
• Collaborate with the care team to develop and follow care plans
• Support activities of daily living and promote patient independence
• Assist with admissions, discharges, and smooth transitions of care
• Maintain infection control standards and safe practice protocols

Qualifications

• Current LPN license (required) — ability to practice in Pennsylvania preferred
• Current BLS/CPR certification (or willingness to obtain promptly)
• Up-to-date immunizations and ability to pass background and drug screening
• Comfortable working overnight shifts in a long-term care setting
• Strong communication skills and a team-oriented attitude
• Previous experience in long-term care or skilled nursing is a plus

Education

• Graduate of an accredited LPN / practical nurse program
• Current LPN licensure in good standing

Knowledge & Skills

• Medication administration and basic clinical assessments
• Charting and electronic health record familiarity
• Wound care basics and infection prevention practices
• Time management for busy shift workflows
• Effective communication with residents, families, and peers
