Roles & Responsibilities
- Provide high-quality, compassionate direct patient care within the LPN scope of practice
- Administer medications and treatments accurately while following facility policies
- Monitor and document patient status, vital signs and care interventions in the chart
- Collaborate with RNs, CNAs and interdisciplinary team members to support care plans
- Communicate clearly and compassionately with patients and their families
- Assist with admissions, discharges and transitions to ensure continuity of care
- Maintain a safe, clean environment and adhere to infection control standards
- Participate in facility initiatives to improve patient outcomes and resident satisfaction
Qualifications
- Current Licensed Practical Nurse (LPN) license valid in Pennsylvania
- Current CPR/BLS certification (or willingness to obtain upon hire)
- Comfortable administering medications and performing nursing procedures per LPN scope
- Strong communication and teamwork skills to collaborate with clinical staff
- Preferred: experience in long-term care, skilled nursing or similar settings
Education
- Completion of an accredited LPN/LVN program and active LPN license
Knowledge & Skills
- Medication administration and basic wound care knowledge
- Familiarity with charting and electronic medical records
- Basic assessment skills and ability to recognize changes in patient condition
- Infection control best practices and safe patient handling
- Time management, prioritization and the ability to multitask in a busy environment
- Compassionate bedside manner and strong interpersonal skills
