Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at the facility
• Administer medications and treatments accurately and document per policy
• Perform thorough patient assessments and report changes promptly
• Collaborate with the interdisciplinary team to develop and update care plans
• Educate patients and families about care plans, medications, and recovery steps
• Maintain clear, timely charting in the facility’s documentation system
• Support infection control and safety protocols to protect patients and staff

Qualifications

• Current LPN license (Licensed Practical Nurse) — Pennsylvania license preferred
• CPR/BLS certification required; ACLS or additional certifications a plus
• Prior experience in long-term care, skilled nursing, or similar settings preferred
• Strong communication skills and ability to work collaboratively with clinical teams
• Reliable, professional, and committed to delivering high-quality patient care

Education

• Practical Nursing diploma, certificate, or equivalent training required
• High school diploma or GED required

Knowledge & Skills

• Medication administration and documentation best practices
• Fundamentals of wound care and basic procedures
• Vital signs monitoring and basic patient assessment
• Electronic health record (EHR) charting and clinical documentation
• Time management, prioritization, and multitasking in a clinical setting
• Patient education, empathy, and effective family communication
