Roles & Responsibilities
- Provide compassionate direct care to residents, assisting with daily living activities
- Administer medications and treatments as ordered, following facility protocols
- Perform routine assessments and promptly report changes in patient condition
- Document care accurately and timely in the resident record
- Collaborate with nurses, therapists, and interdisciplinary team members to support resident goals
- Assist with admissions, transfers, and discharge processes when needed
- Maintain a safe, clean environment and adhere to infection control practices
Qualifications
- Valid LPN license (Licensed Practical Nurse) — required
- Completion of an accredited practical nursing program
- Current BLS/CPR certification (or willingness to obtain)
- Ability to pass background checks and meet facility health requirements
- Strong communication and teamwork skills with a resident-centered approach
Education
- Completion of an accredited LPN (Practical Nursing) program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Comfortable with routine assessments, documentation, and care planning
- Knowledge of infection control and safety standards
- Proficient in communicating with residents, families, and team members
- Organized, reliable, and attentive to detail in clinical tasks
