Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and routine assessments
• Monitor and document vital signs, patient status changes, and care interventions accurately
• Collaborate with the interdisciplinary team to develop and carry out individualized care plans
• Support residents with activities of daily living while promoting dignity and independence
• Communicate clearly and respectfully with patients, families, and team members to ensure continuity of care
• Report concerns and escalate changes in condition promptly to RN leadership and providers
• Maintain accurate, timely documentation in the facility record system

Qualifications

• Current Licensed Practical Nurse (LPN) license (required)
• State LPN licensure in Pennsylvania (or ability to obtain prior to start)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Experience in long-term care or skilled nursing settings preferred
• Strong communication skills and the ability to work collaboratively with a diverse care team

Education

• Graduate of an accredited LPN/Licensed Vocational Nurse program
• High school diploma or equivalent

Knowledge & Skills

• Solid knowledge of medication administration and infection control practices
• Comfortable performing wound care, ADL assistance, and routine clinical assessments
• Skilled in clear, timely documentation and charting
• Ability to prioritize tasks and manage time during busy night shifts
• Empathetic bedside manner and strong patient/family communication
