Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and safely
- Monitor patient conditions and report changes promptly to the care team
- Collaborate with nurses, therapists, and support staff to create individualized care plans
- Document patient care activities clearly and timely in the chart
- Support residents' activities of daily living while promoting dignity and independence
- Participate in interdisciplinary team meetings to improve patient outcomes
Qualifications
- Active Licensed Practical Nurse (LPN) license — ready to practice in the required state
- Current Basic Life Support (BLS) certification or willingness to obtain
- Comfortable working a full clinical shift and providing direct patient care
- Strong communication skills and a team-first attitude
- Previous long-term care or skilled nursing experience preferred but not required
Education
- Graduation from an accredited LPN/LVN program or equivalent practical nurse training
- High school diploma or GED required
Knowledge & Skills
- Solid understanding of basic nursing procedures and medication administration
- Ability to assess patient needs and escalate concerns appropriately
- Good documentation skills and familiarity with clinical charting
- Comfortable working with elderly populations and chronic-care needs
- Team collaboration, time management, and compassionate bedside manner
