Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents while promoting dignity and independence
- Administer medications and treatments accurately and document in the medical record
- Monitor patient status and communicate changes promptly to the care team
- Assist with daily living activities and create a safe, comfortable environment
- Collaborate with nurses, therapists and physicians to carry out individualized care plans
- Participate in shift handoffs and maintain clear, timely documentation
Qualifications
- Active Licensed Practical Nurse (LPN) license (Pennsylvania preferred or compact where applicable)
- Current BLS/CPR certification preferred
- Demonstrated ability to provide safe, patient-centered care in a long-term care or skilled nursing setting
- Reliable, compassionate, and able to communicate effectively with residents and team members
- Recent clinical experience a plus, but dedication and a willingness to learn matter
Education
- LPN diploma or vocational nursing certificate from an accredited program
Knowledge & Skills
- Proficient in medication administration and basic clinical assessments
- Strong documentation and charting skills
- Knowledge of infection control and safety procedures
- Ability to prioritize tasks during a busy 12+ hour shift
- Excellent interpersonal skills and team collaboration
