Roles & Responsibilities
- Provide compassionate direct patient care, including assistance with activities of daily living
- Administer medications and treatments according to orders and facility protocols
- Monitor and document vital signs, patient status changes, and clinical observations
- Collaborate with RNs and interdisciplinary team members to support individualized care plans
- Perform routine wound care and follow infection control standards
- Communicate clearly with patients and families to educate and support their needs
- Accurately maintain medical records and electronic charting in a timely manner
- Support a safe, respectful environment and escalate concerns appropriately
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
- Current BLS/CPR certification (preferred or obtain upon hire)
- Experience in long-term care, rehabilitation, or skilled nursing settings is a plus
- Up-to-date immunizations and TB screening per facility policy
- Strong communication skills and a patient-first mindset
- Ability to work overnight shifts and collaborate with a multidisciplinary team
Education
- Completion of an accredited LPN practical nursing program; active PA LPN license required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic clinical assessment and vital sign interpretation
- Infection prevention and universal precautions
- Proficiency with electronic medical records or clinical charting
- Time management and prioritization in a fast-paced care setting
- Effective patient and family education and communication
