Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and follow prescribed treatment plans accurately
• Monitor patient status and report clinical changes to the care team promptly
• Maintain clear, timely documentation in patient charts and electronic records
• Support activities of daily living while promoting dignity and independence
• Collaborate with nurses, therapists and other staff to deliver coordinated care

Qualifications

• Current Licensed Practical Nurse (LPN) licensure (required)
• Previous experience in long-term care or similar settings is a plus
• Valid basic certifications as required by the facility (for example, BLS/CPR if requested by employer)
• Strong communication skills and a team-focused attitude

Education

• Diploma or certificate from an accredited practical nursing program

Knowledge & Skills

• Medication administration and basic wound care
• Patient assessment and vital signs monitoring
• Accurate charting and familiarity with clinical documentation
• Infection control practices and safe patient handling
• Compassionate resident-centered care and effective teamwork
