Roles & Responsibilities
- Provide compassionate direct patient care to residents based on individualized plans
- Administer medications and treatments safely and document all interventions
- Perform routine assessments, monitor patient status, and report changes to the care team
- Collaborate with nurses, physicians, and interdisciplinary staff to coordinate care
- Maintain accurate, timely documentation in patient records and electronic systems
- Support admissions, discharges, and transfers to ensure continuity of care
- Promote a safe, clean environment and adhere to infection control practices
- Educate residents and families about care plans, medications, and self-care
Qualifications
- Current LPN licensure (Pennsylvania licensure strongly preferred)
- Completion of an accredited Practical Nursing program
- Current BLS/CPR certification (or willingness to obtain)
- Recent hands-on LPN experience in long-term care, skilled nursing, or similar setting preferred
- Reliable communication skills and a team-oriented attitude
Education
- Diploma or certificate from an accredited Practical Nursing (LPN) program
Knowledge & Skills
- Skilled in medication administration and basic clinical procedures
- Strong resident assessment and documentation abilities
- Familiarity with wound care and basic restorative nursing techniques
- Comfortable working within an interdisciplinary team
- Basic computer/EHR navigation skills for charting and reporting
