Roles & Responsibilities
- Provide high-quality, compassionate nursing care to residents as an LPN
- Administer medications and document treatments accurately and timely
- Monitor resident conditions and communicate changes to the care team
- Assist with activities of daily living and promote resident dignity
- Collaborate with RNs, therapists, and support staff to create meaningful care plans
- Maintain thorough, compliant clinical documentation and participate in care meetings
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
- Current BLS (Basic Life Support) certification required or obtainable upon hire
- Previous experience in long-term care or skilled nursing settings preferred
- Current immunizations and the ability to pass background and health screenings
- Strong communication skills and a patient-centered attitude
Education
- Completion of an accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Medication administration and accurate charting
- Basic nursing assessments and vital sign monitoring
- Familiarity with long-term care workflows and documentation
- Ability to work collaboratively in an interdisciplinary team
- Time management, prioritization, and compassionate bedside manner
