Roles & Responsibilities

• Provide compassionate, hands-on care to residents including assistance with activities of daily living
• Monitor and record vital signs and report changes in condition promptly
• Support mobility, positioning, and safe transfers while promoting resident independence
• Maintain accurate and timely documentation in the patient record
• Collaborate with nurses and interdisciplinary team members to deliver coordinated care
• Follow infection control practices and facility policies to ensure a safe environment
• Assist with meal service and feeding support as needed, honoring dietary requirements
• Build trusting relationships with residents and families through respectful communication

Qualifications

• Current Licensed Nursing Assistant (LNA) certification (NH required; VT endorsement helpful for dual-duty areas)
• BLS/CPR certification preferred or willingness to obtain upon hire
• Previous experience in long-term care, skilled nursing, or similar settings is a plus
• Up-to-date immunizations and ability to pass background checks as required
• Dependable, compassionate, and able to work a rotating night/weekend schedule
• Good communication skills and ability to work collaboratively with clinical teams

Education

• High school diploma or GED
• Completed LNA training program and current LNA license

Knowledge & Skills

• Comfortable performing ADLs, toileting, feeding, and mobility assistance
• Ability to take and accurately record vital signs and observe changes
• Basic knowledge of infection prevention and safe patient handling
• Strong time management and prioritization skills during busy shifts
• Clear, professional communication with residents, families, and team members
• Familiarity with electronic charting or willingness to learn facility EMR systems
