Shift Type: Night shift, 11:00 PM - 7:30 AM NOC; schedule varies, every-other-weekend required

Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Provide skilled, compassionate nursing care to residents in a rehabilitation and long-term care setting
- Administer medications and treatments accurately and on schedule
- Assess and monitor patient status, documenting changes clearly in the medical record
- Support rehabilitation plans by assisting with activities of daily living and mobility
- Collaborate with nurses, therapists, and physicians to deliver coordinated care
- Maintain a safe, clean environment and follow infection control best practices
- Communicate effectively with patients and families to educate and provide reassurance
- Participate in shift handoffs and contribute to continuous quality improvement
Qualifications
- Active Licensed Practical Nurse (LPN) license eligible for practice in Vermont
- Current BLS/CPR certification (or able to obtain prior to start)
- Prior experience in long-term care, rehabilitation, or similar clinical setting preferred
- Comfortable working night shifts and covering every-other-weekend requirements
- Strong clinical judgment, dependable attendance, and a patient-centered attitude
- Ability to pass facility onboarding screenings and background checks
Education
- Practical Nursing diploma, certificate, or equivalent from an accredited program
Knowledge & Skills
- Proficient in medication administration and basic nursing procedures
- Skilled in patient assessment and monitoring of vital signs
- Experienced with documentation and electronic medical records
- Knowledge of wound care basics and safe patient handling
- Excellent communication, teamwork, and time-management skills
- Ability to prioritize tasks in a fast-paced rehab/long-term care environment
