Roles & Responsibilities

• Provide skilled, compassionate nursing care to residents in a rehabilitation and long-term care setting
• Administer medications and treatments accurately and on schedule
• Assess and monitor patient status, documenting changes clearly in the medical record
• Support rehabilitation plans by assisting with activities of daily living and mobility
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Maintain a safe, clean environment and follow infection control best practices
• Communicate effectively with patients and families to educate and provide reassurance
• Participate in shift handoffs and contribute to continuous quality improvement

Qualifications

• Active Licensed Practical Nurse (LPN) license eligible for practice in Vermont
• Current BLS/CPR certification (or able to obtain prior to start)
• Prior experience in long-term care, rehabilitation, or similar clinical setting preferred
• Comfortable working night shifts and covering every-other-weekend requirements
• Strong clinical judgment, dependable attendance, and a patient-centered attitude
• Ability to pass facility onboarding screenings and background checks

Education

• Practical Nursing diploma, certificate, or equivalent from an accredited program

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Skilled in patient assessment and monitoring of vital signs
• Experienced with documentation and electronic medical records
• Knowledge of wound care basics and safe patient handling
• Excellent communication, teamwork, and time-management skills
• Ability to prioritize tasks in a fast-paced rehab/long-term care environment
