Roles & Responsibilities
- Provide hands-on personal care following each patient’s individualized plan of care
- Perform vital sign checks and document findings accurately and timely
- Assist with ambulation, transfers and safe positioning to promote mobility and prevent pressure injuries
- Support infection prevention and control practices consistently
- Observe and report changes in patient condition to the RN promptly
- Maintain clear, timely documentation in the EMR per policy and state requirements
- Build positive, respectful relationships with patients and families and honor patient rights
- Work as a collaborative member of the interdisciplinary care team
Qualifications
- Active Michigan CNA license required (must be active!)
- Certified Nursing Assistant (CNA) certification
- BLS (AHA) certification required — please include documentation
- Minimum 2+ years of Long-Term Care (LTC) experience required
- Demonstrated competency in Microsoft Windows and comfort with Epic EMR
- Submission packet must include work history, current (≤1 year) skills checklist, and 2 supervisory reference forms (taken within last 2 years)
- Candidates must provide permanent address and cannot be local within a 100-mile radius (facility restriction)

- Patient-centered hands-on care and strong interpersonal communication
- Accurate measurement and documentation of vital signs
- Infection prevention and control best practices
- Pressure prevention, skin integrity care and proper positioning techniques
- Safe ambulation and transfer assistance
- Timely, policy-compliant documentation in Epic EMR
- Comfortable using Microsoft Windows-based systems
