Roles & Responsibilities

• Assess client health needs and determine individualized nursing care requirements for assigned home health patients
• Plan and direct nursing care, coordinating care plans with other nursing staff and the interdisciplinary team
• Evaluate the effectiveness of care plans and make timely adjustments to improve outcomes
• Deliver competent nursing care across the lifespan — neonate, pediatric, adolescent, adult and geriatric clients
• Document care thoroughly and accurately in Epic and other required systems
• Provide clinical oversight and direction to other nursing staff during client visits
• Ensure safe travel to patient homes and maintain required safety and documentation standards

Qualifications

• Active Michigan RN license (must be active)
• Minimum 1 year of Home Care RN experience (required)
• Current American Heart Association BLS certification (required)
• Active driver’s license, reliable vehicle, auto insurance and a clean DMV report (required)
• Experience with IV therapy, wound vacs and central line care highly preferred
• MI LTC fingerprinting required during compliance process
• Two supervisory reference forms completed within the last two years (required)
• Up-to-date Skills Checklist relevant to Home Health (within 1 year) — required

Knowledge & Skills

• Patient assessment and development of individualized care plans
• Care evaluation and clinical decision-making across age groups
• Nursing care delivery in the home setting with strong documentation practices
• Directing and mentoring nursing staff and coordinating interdisciplinary care
• Experience with Epic electronic medical record
• IV therapy, wound vacs and central line care
• Strong time management, travel planning and infection control practices for home visits
