Roles & Responsibilities

• Coordinate patient care activities as an RN Case Manager to ensure safe transitions and optimal outcomes
• Assess patient needs, create individualized care plans, and collaborate with interdisciplinary teams
• Communicate clearly with patients, families, and clinical staff to facilitate timely discharges and referrals
• Document case management activities accurately and timely in the facility’s systems
• Verify patients’ eligibility and resources and connect them with appropriate community services
• Participate in competency assessments including Prime Core Competencies Exam and Prime Skills Checklist

Qualifications

• Active RN license in accordance with California state requirements (must provide license verification)
• Certifications as required by unit: BLS required; ACLS, PALS, NRP depending on unit
• Compliance with health screening and immunization requirements (COVID-19 vaccine, MMR, Varicella or declination, Influenza, TB testing, Tdap, Hepatitis B where applicable)
• Successful completion of Prime Core Competencies Exam and Prime Skills Checklist
• Negative drug screening (minimum 10-panel) and a physical/health statement indicating ability to work without restrictions
• Ability to commit to the 26-week per diem assignment and meet minimum compliance deadlines (minimum compliance due by 08/10/2025)

• Strong case management and care coordination skills
• Excellent clinical assessment and critical thinking for acute care settings
• Clear, compassionate communication with patients, families, and care teams
• Proficient documentation and electronic health record usage
• Ability to work independently as per diem staff and adapt to changing unit needs
• Familiarity with credentialing and compliance processes for per diem/contract staff
