Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in the CVN Crestview Center
• Administer medications and treatments safely and accurately, following facility protocols
• Perform thorough assessments and document clinical findings clearly in the patient record
• Collaborate with RNs, physicians, and interdisciplinary team members to develop and adjust care plans
• Monitor resident condition and promptly report changes to the appropriate clinical staff
• Support daily living activities and promote dignity, independence, and quality of life for residents
• Participate in shift handoffs and maintain clear, timely communication with the care team

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Graduation from an accredited practical nursing program
• Current BLS/CPR certification (preferred or required depending on facility policy)
• Comfortable working in long-term care / CVN environment and providing hands-on resident care
• Strong communication skills and a team-first attitude
• Ability to pass background check, drug screen, and any required health screenings

Education

• Completion of an accredited Practical Nursing (LPN) program or vocational nursing certificate

Knowledge & Skills

• Hands-on experience with medication administration and documentation
• Basic assessment skills: vital signs, wound checks, and monitoring clinical changes
• Familiarity with electronic health record (EHR) charting and clear clinical documentation
• Knowledge of infection control practices and safe patient handling
• Excellent interpersonal skills for working with residents, families, and the care team
• Ability to prioritize, multitask, and stay calm under pressure
