Roles & Responsibilities
- Assess speech, language, cognition, and swallowing function for geriatric patients
- Develop and implement individualized treatment plans to achieve meaningful outcomes
- Document evaluations, progress notes, and discharge summaries accurately and timely
- Collaborate with nurses, therapists, physicians, and family members as part of the care team
- Provide patient and caregiver education to support carryover and functional independence
- Participate in interdisciplinary team meetings and contribute to discharge planning
- Ensure compliance with facility policies, regulatory requirements, and best practices
Qualifications
- Active state license as a Speech-Language Pathologist (required prior to start)
- ASHA CCC-SLP or working toward certification preferred
- Experience or strong interest in geriatric/rehabilitation populations
- Proven ability to produce timely, thorough clinical documentation
- Strong communication and teamwork skills in interdisciplinary settings
- Current Basic Life Support (BLS) preferred or willing to obtain
Education
- Master's degree in Speech-Language Pathology or equivalent from an accredited program
Knowledge & Skills
- Skilled in dysphagia assessment and management for older adults
- Knowledge of evidence-based speech, language, and cognitive therapy techniques
- Familiarity with electronic medical record documentation and clinical workflow
- Ability to adapt treatment approaches to varied functional goals and settings
- Comfortable coaching caregivers and family members to support progress
