Roles & Responsibilities

• Assess speech, language, cognition, and swallowing function for geriatric patients
• Develop and implement individualized treatment plans to achieve meaningful outcomes
• Document evaluations, progress notes, and discharge summaries accurately and timely
• Collaborate with nurses, therapists, physicians, and family members as part of the care team
• Provide patient and caregiver education to support carryover and functional independence
• Participate in interdisciplinary team meetings and contribute to discharge planning
• Ensure compliance with facility policies, regulatory requirements, and best practices

Qualifications

• Active state license as a Speech-Language Pathologist (required prior to start)
• ASHA CCC-SLP or working toward certification preferred
• Experience or strong interest in geriatric/rehabilitation populations
• Proven ability to produce timely, thorough clinical documentation
• Strong communication and teamwork skills in interdisciplinary settings
• Current Basic Life Support (BLS) preferred or willing to obtain

Education

• Master's degree in Speech-Language Pathology or equivalent from an accredited program

Knowledge & Skills

• Skilled in dysphagia assessment and management for older adults
• Knowledge of evidence-based speech, language, and cognitive therapy techniques
• Familiarity with electronic medical record documentation and clinical workflow
• Ability to adapt treatment approaches to varied functional goals and settings
• Comfortable coaching caregivers and family members to support progress
