Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, ensuring dignity and comfort
• Administer medications and treatments accurately and safely according to orders
• Monitor and document patient status changes promptly in the chart
• Collaborate with interdisciplinary team members to develop person-centered care plans
• Perform thorough assessments and communicate findings to nursing leadership and providers
• Promote a safe, clean environment and adhere to infection control and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license (WV licensure preferred or ability to obtain prior to start)
• Current Basic Life Support (BLS/CPR) certification or ability to obtain
• Comfortable working independently on overnight shifts and escalating concerns appropriately
• Strong attention to detail and solid documentation skills
• Positive attitude, teamwork mindset, and commitment to resident-centered care

Education

• Graduate of an accredited practical nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Clinical assessment and basic nursing interventions for adult and geriatric populations
• Medication administration and safe charting practices
• Wound care basics and monitoring of chronic conditions
• Familiarity with electronic health records and accurate documentation
• Infection control practices and safety protocols
• Strong verbal communication and collaboration with interdisciplinary teams
