Roles & Responsibilities

• Provide patient-centered physical therapy evaluations and develop individualized treatment plans
• Implement therapeutic interventions to improve mobility, strength, and function
• Document clinical progress clearly and timely in the patient record
• Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
• Educate patients and families on home programs and strategies for long-term success
• Monitor patient response and adjust treatment plans based on clinical findings
• Participate in quality improvement initiatives to enhance patient care

Qualifications

• Licensed Physical Therapist in Pennsylvania or eligible for licensure
• Doctor of Physical Therapy (DPT) or equivalent degree preferred
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Comfortable working as part of a multidisciplinary team and communicating with patients and families
• Current immunizations and ability to pass standard background and health screenings as required

Education

• Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy considered
• State PT license required or eligible

Knowledge & Skills

• Skilled in patient evaluation, therapeutic exercise, and manual therapy techniques
• Strong clinical reasoning and documentation skills
• Ability to develop individualized treatment plans and adjust care based on progress
• Good interpersonal skills and the ability to educate patients and caregivers
• Familiarity with common outpatient and post-acute diagnoses and functional goals
