Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Assist with safe transfers, mobility support and fall prevention measures
• Perform routine tasks such as vital signs, feeding assistance, and toileting support
• Maintain accurate documentation and communicate effectively with interdisciplinary staff
• Follow infection control protocols and contribute to a clean, safe care environment
• Support residents' emotional and social needs by offering respectful, person-centered care

Qualifications

• Current Certified Nursing Assistant (CNA) credential required
• T-CNA certification preferred (as indicated in the job posting)
• Previous CNA experience in long-term care or rehab settings is a plus
• Current Basic Life Support (BLS/CPR) certification preferred
• Ability to pass background check and required pre-placement screenings
• Strong communication skills and a team-oriented approach

Education

• High school diploma or GED
• Completion of an approved CNA training program

Knowledge & Skills

• Comfortable assisting with ADLs (bathing, dressing, grooming, feeding)
• Ability to take and document vital signs accurately
• Knowledge of infection prevention and safe patient handling
• Familiarity with basic charting and electronic medical records
• Compassionate bedside manner and strong observational skills
