Roles & Responsibilities

• Deliver skilled physical therapy services to home health and hospice patients across the northern NH territory
• Perform comprehensive evaluations and create individualized treatment plans for patients from pediatrics through geriatrics
• Complete accurate OASIS documentation and maintain compliance with home health regulatory standards
• Coordinate care with nurses, OTs, SLPs, social workers and caregivers to maximize patient independence
• Educate patients and families on safe mobility, fall prevention, and home exercise programs
• Provide compassionate palliative and hospice-focused therapy when appropriate, supporting quality of life
• Maintain timely, professional documentation and submit required supervisory references and checklists
• Safely travel between patient homes — plan routes and adapt to winter driving conditions

Qualifications

• Active New Hampshire Physical Therapy license (Compact allowed) — required
• Minimum 2+ years current experience in Physical Therapy in Home Health — required
• Home Health and/or Hospice experience with OASIS proficiency — required
• Active American Heart Association BLS (AHA) — required
• Comfortable driving your own vehicle in winter conditions and able to travel across the northern territory
• Pediatric to geriatric age-group experience — able to treat a broad caseload
• Supervisory signed reference from the past year and a current PT skills checklist — required
• Permanent address must be included in submission; NO LOCALS — candidate must reside 50+ miles from facility

Knowledge & Skills

• Strong knowledge of OASIS documentation and home health compliance
• Skilled in mobility assessment, balance/fall prevention, and therapeutic exercise
• Experience providing care across the lifespan from pediatric to geriatric patients
• Excellent patient education, communication, and interdisciplinary coordination skills
• Time management and caseload prioritization for efficient travel-based care
• Comfort with electronic medical records and timely documentation
• Ability to adapt treatment plans in home environments and to work independently
