Roles & Responsibilities
- Provide compassionate, hands-on care to residents in a skilled nursing facility
- Assist with activities of daily living including bathing, dressing, grooming, and mobility
- Monitor and document resident vital signs and condition changes accurately
- Follow care plans and communicate resident needs to the nursing team
- Support infection control practices, including adherence to fit testing and PPE requirements
- Complete facility-specific competency tasks and required clinical checks
- Help create a safe, respectful, and comfortable environment for residents
Qualifications
- Active CNA certification in the relevant state
- Current BLS certification
- License or certificate verification completed during onboarding
- Completion of required Trinity Specialty Specific Competency Testing
- Must meet health and screening requirements: Fit Test; 2-step PPD or IGRA; immunizations including MMR, Hepatitis B, Varicella, Tdap, Influenza; COVID-19 vaccine as required
- Negative Trinity 10 Panel Drug Screen and completed physical/health statement
- Previous experience in long-term care or skilled nursing preferred
Education
- State-approved CNA certification
- High school diploma or equivalent preferred
Knowledge & Skills
- Strong resident-focused bedside care skills
- Clear communication and teamwork with nursing staff and interdisciplinary teams
- Ability to document care accurately and follow care plans
- Infection prevention and control practices
- Comfortable assisting with mobility and transfers
- Attention to detail for competency testing and health screening compliance
