Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a skilled nursing facility
- Administer medications and treatments safely and according to facility protocols
- Monitor resident condition, record vital signs, and promptly report changes
- Collaborate with nurses, therapists, and interdisciplinary team members to support care plans
- Maintain accurate, timely documentation in the resident record
- Support residents with activities of daily living while promoting dignity and independence
- Participate in infection control, safety practices, and facility quality initiatives
Qualifications
- Licensed Practical Nurse (LPN) — active state license and successful license verification required
- BLS (Basic Life Support) certification required
- Completion of Trinity Specialty Specific Competency Testing as required
- Ability to meet health screening and immunization requirements: Fit Test, 2-step PPD or IGRA, Influenza vaccine, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine
- Negative Trinity 10 Panel Drug Screen and completed Physical/Health Statement
- Trinity Respirator Evaluation and Trinity Color Vision as required by the facility
- Prior experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred
Education
- Graduate of an accredited LPN program or equivalent
- Current LPN license and BLS certification
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Familiarity with SNF standards, infection control, and safety procedures
- Comfortable using electronic health records and accurate documentation practices
- Good communication and teamwork skills with residents, families, and staff
- Ability to prioritize, multitask, and respond calmly in urgent situations
- Compassionate, resident-focused approach with attention to dignity and person-centered care
