Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing facility
• Administer medications and treatments safely and according to facility protocols
• Monitor resident condition, record vital signs, and promptly report changes
• Collaborate with nurses, therapists, and interdisciplinary team members to support care plans
• Maintain accurate, timely documentation in the resident record
• Support residents with activities of daily living while promoting dignity and independence
• Participate in infection control, safety practices, and facility quality initiatives

Qualifications

• Licensed Practical Nurse (LPN) — active state license and successful license verification required
• BLS (Basic Life Support) certification required
• Completion of Trinity Specialty Specific Competency Testing as required
• Ability to meet health screening and immunization requirements: Fit Test, 2-step PPD or IGRA, Influenza vaccine, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine
• Negative Trinity 10 Panel Drug Screen and completed Physical/Health Statement
• Trinity Respirator Evaluation and Trinity Color Vision as required by the facility
• Prior experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred

Education

• Graduate of an accredited LPN program or equivalent
• Current LPN license and BLS certification

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Familiarity with SNF standards, infection control, and safety procedures
• Comfortable using electronic health records and accurate documentation practices
• Good communication and teamwork skills with residents, families, and staff
• Ability to prioritize, multitask, and respond calmly in urgent situations
• Compassionate, resident-focused approach with attention to dignity and person-centered care
