Roles & Responsibilities
- Provide compassionate, skilled daily treatments to residents in a skilled nursing facility
- Document accurate and timely progress notes for each patient encounter
- Collaborate closely with the therapy team to create and implement effective care plans
- Meet productivity expectations and work efficiently to support patient outcomes (facility productivity target 92%)
- Follow Continuum Rehabilitation Group (CRG) policies and procedures to ensure compliance and quality
- Maintain professional presentation and follow dress code (black scrubs) while representing the facility
Qualifications
- Licensed Physical Therapist Assistant (PTA) in good standing
- 1-3 years of experience preferred (job lists 1-3 years; prior SNF experience strongly preferred)
- Experience working in a skilled nursing facility (SNF) strongly preferred
- Ability to complete daily treatments and timely progress notes
- Willingness to be on-site to receive guaranteed hours and to follow facility policies
Knowledge & Skills
- Skilled in delivering daily PTA treatments in a SNF environment
- Strong documentation skills for progress notes and treatment records
- Comfortable working within a multidisciplinary therapy team
- Familiarity with SNF workflows, productivity expectations, and regulatory requirements
- Professional communication and patient-centered care approach
